Default Heroic Mode
While in combat I engaged a :normal: mob. Midway in combat a rank 20 spell TP sold in the AH. Shortly thereafter I was pulled form combat into the arena.


After the arena I engaged a fungi mob and it was "heroic". I looked back and never saw a misclicked "Heroic mode activated" message. I verified the floating and hard text when this option is selected.

Were you farming alone or were you group farming?
